CONTACT FOR MEDICATION MANAGEMENT
For Prescription Refills: Please contact your pharmacy to request a refill of your medication. Your pharmacy will fax the request to your provider. If you are taking a medication that requires a paper prescription (controlled substance) to be picked up in our office, please contact medmgtinfo@collegesupportnw.com. Please allow up to 72 hours for refill requests to be submitted as this cannot be done after hours. Controlled substances cannot be refilled if you have missed or cancelled an appointment that corresponds with the prescription refill.
